I have a DVD booklet (reader spread) 4.75in x 7.25in which I have exported as a pdf. The problem that I'm having is that when I print the document as 8.5 x 11 page with the booklet printing option in Acrobat Reader the pages width become 5in instead of 4.75. Any idea how I can keep the width as 4.75in.

Check the printer setup dialog from within Print Booklet. It sounds like either you've added bleed or set a custom page size larger than you intended.
